3/19/2023 EnchantaVaughn Approved Behavior: At the start of session, client expressed negative “I statements” (e.g. "I hate staying in this boring house"). Client would not own up to anything and continued to look clueless in the face. Client finally opened up about what happened and was honest and upfront to QMHP. Once client appeared to be calm, she began explaining how she felt disrespected due to her mom yelling at her earlier. Client apologized to her mom and switched to a more outgoing mood. Client is learning to respect authority figures. Client promised to be more respectful and comply to rules. Behavior: Client had difficulty concentrating on the therapeutic activity as she continued to disrupt everyone in the house. Client was reminded that she was not setting a good example for her younger brother. Client did not demonstrate a cooperative attitude during the family activity. Client was provided with feedback about how her behavior and verbal messages have a huge impact on the people who love and care for her. Intervention: QMHP encouraged client to listen to her mother. QMHP encouraged client to be the example for her little brother. QMHP encouraged client not to follow others but to be her own person. QMHP will provide strategies for coping and self-control when faced with conflict. QMHP will review and give feedback regarding ADHD and its causes and effect to well-being and social performance. QMHP assessed and monitored client’s mood and behavior to see which interventions would be the most useful. Intervention: QMHP focused the next part of the session on limiting rivalry in the home. QMHP assisted client with recognizing and exploring triggers of her impatience. QMHP encouraged client to continue documenting people and situations that evoke anger or aggressive behaviors. QMHP utilized active listening skills while probing client about situations that provokes anger. After processing client’s feelings, she was redirected to continue exploring feelings and ways to conduct herself appropriately when she becomes upset. Response: Client stated she would try better self-management as the day pursues. Once she was able to calm down, client showed excellent cooperation and treating her family with love and respect. Client was able to complete her therapeutic activity with less than 3 prompts to redirect her focus in the home. Client showed an improvement from previous defiant experiences in the home with mom. Client learned to avoid negative and defiant behavior and comply to rules. Response: Client was able to maintain her composure when she was confronted with the potential for conflict. Client was observed engaging in several conflict resolution skills by walking away and telling an adult first. Client verbalized that she’s able to focus better when it is quieter and more peaceful. Client took her mind from thinking and doing to a place of concentration, which allowed her to focus on one thing at a time. Client remembered to apply herself and utilize her strengths to focus on meeting her goals and objectives.
